The Danish Health Authority has initiated a central health reform task aimed at designing a generic model for chronic disease pathways, covering conditions such as type 2 diabetes, heart disease, and complex multimorbidity. The model will standardize coordination across sectors while emphasizing general practice as a primary coordinator. Digital solutions and data use will be integrated to support individualized management and better disease monitoring. The work is part of a broader reform agreed in 2024 and scheduled to take full effect from 2027. Stakeholders including regions, municipalities, and patient organizations are involved in follow‑up groups to shape implementation and ensure equitable access to treatment nationwide.
